Ventilation-perfusion matching, not functional residual capacity, should be used to determine oxygenation with PEEP.

The pulse oximeter, which is convenient though not ideal, can be useful in bedside manipulation and titration of PEEP under these circumstances when hypoxemia is refractory or deteriorating. Ofcourse, adequate oxygen transport should be the end point.

Application of positive end expiratory pressure during laparoscopic surgery
provided the original work is properly cited. CC Positive-pressure ventilation during general anesthesia is a prerequisite for numerous surgical conditions. Mechanical ventilation is non-physiological and can induce lung injury, although ventilator-induced lung injury may be of minor clinical importance during anesthesia for most patients with healthy lungs. However, growing evidence suggests t...
متن کاملIn prone ventilation, one good turn deserves another.
The management of the acute respiratory distress syndrome (ARDS) remains daunting as we approach a half century since its description.1 Both direct and indirect insults produce acute lung injury, diffuse alveolar damage, and increased vascular permeability, with more severe involvement classified as ARDS.2 Although this suggests homogeneous injury, computed tomography clearly shows heterogeneou...
متن کاملThe effect of positive-end expiratory pressure on oxygenation during high frequency jet ventilation and conventional mechanical ventilation in the rabbit model of acute lung injury
BACKGROUND The use of positive end expiratory pressure (PEEP) in patients with acute lung injury (ALI) improves arterial oxygenation by alleviating pulmonary shunting, helping the respiratory muscles to decrease the work of breathing, decreasing the rate of infiltrated and atelectatic tissues, and increasing functional residual capacity. In a rabbit model of saline lavage-induced ALI, we examin...
متن کاملShould PEEP Titration Be Based on Chest Mechanics in Patients With ARDS?
Functional residual capacity (FRC) is essentially the alveolar volume and a determinant of both oxygenation and respiratory system compliance (CRS). ARDS decreases FRC, and sufficient PEEP restores FRC; thus, assessments of PEEP by its impact on oxygenation and CRS are intimately linked. PEEP also can ameliorate or aggravate ventilator-induced lung injury. Therefore, it can be argued that PEEP ...
متن کاملAssessment of Lung Recruitment by Electrical Impedance Tomography and Oxygenation in ARDS Patients
We hypothesized that not all patients with appreciably recruited lung tissue during a recruitment maneuver (RM) show significant improvement of oxygenation. In the present study, we combined electrical impedance tomography (EIT) with oxygenation measurements to examine the discrepancies of lung ventilation and perfusion versus oxygenation after RM.A 2-minute RM (20 cm H2O positive end-expirator...
